Each of these forms must be completed by you and your child's pediatrician. These forms are required to be kept on file by the Maryland State Department of Education. Please be sure to complete each form with the information required. Make sure that you have signed each form that requires a parent signature. There is also confusion about whether you must have the Lead Screening Form completed. Many parents make the mistake of not having it filled out because they do not reside in one of the zip codes listed. Every child must have a screening done regardless of their place of residence. Families that live in one of the listed zip codes may require further screening, as determined by your doctor.
